Drop in Breast Cancer Death Rates May Not Be Linked to Screening Rates (HealthDay)

FRIDAY, July 29 (HealthDay News) -- Developed countries have seen a drop in breast cancer death rates in recent years, but a new international study suggests this trend is less about rising screening rates and more about the availability of increasingly effective treatments and improving health-care systems.

The finding stems from an analysis of World Health Organization (WHO) breast cancer data collected between 1980 and 2006, in which French, British and Norwegian researchers compared the screening and fatality rates of several Western European countries. The observations were presented online July 29 in the British Medical Journal.

"The contrast between the time differences in implementation of mammography screening and the similarity in reductions in mortality between the country pairs suggest that screening did not play a direct part in the reductions in breast cancer mortality," a team of researchers led by Philippe Autier, research director of the International Prevention Research Institute in Lyon, France, said in a journal news release.

To explore to what degree breast cancer screening rates appeared to be related to fatality rates, the authors chose six countries for points of comparison.

Northern Ireland was stacked up against the Republic of Ireland; the Netherlands was compared with Belgium/Flanders; and Sweden was examined alongside Norway.

The team noted that medical services and the prevalence of breast cancer death risk factors were comparable in each pairing. However, in the space of the study, the second country of each of the three groups was 10 to 15 years behind the first country in terms of their implementation of mammography screening protocols.

The results: Breast cancer death rates, which dropped across the board, were more or less similar in each country pairing, despite stark differences in screening histories.

For example, fatalities between 1989 and 2006 had plummeted by 29 percent in Northern Ireland, as compared with 26 percent in the Republic of Ireland.

The Netherlands registered a 25 percent drop, compared with a 20 percent to 25 percent drop in Belgium/Flanders, while the downward trend of 16 percent in Sweden compared with 24 percent in Norway.

In general terms, Autier's team also noted breast cancer death rates were not that different among women who had undergone almost no screening as compared with those who had been screened quite often.

And the biggest death rate drop seen overall was among women between the ages of 40 and 49, whether or not they had undergone screening.

Dr. Lauren Cassell, chief of breast surgery at Lenox Hill Hospital (affiliated with Northshore/LIJ) in New York City, cautioned that it's not clear that the current findings are easily generalizable to other nations.

"I'm not sure how well this finding would translate in comparison with the U.S.," she noted. "I suspect that we may be more aggressive in our screening protocols, and our health system is very different from the ones they looked at."

"And, in any case, I would certainly say that people should continue getting screened," Cassell stressed. "Because we still do know that catching any cancer at an earlier stage does translate into better outcomes and maybe less treatment. Because even if the end result is the same, and fatal, I certainly would like to have a cancer that could be treated with hormonal treatment and not chemotherapy, if possible."

MM: You use the term ?Queen Yenta of Business Matchmaking? in your Twitter profile. What exactly does that mean?

Eberson: Yenta is a Yiddish term for an old time ?Matchmaker.? Before running the business group, I ran 500 singles? events over five years. My friend coined the name Queen Yenta and the title stuck. Once I started running business events I went from Queen Yenta to Queen Yenta of Business Matchmaking.

MM: Tells us about the NYC Business Networking Group, how it came about and what it does.

Eberson: I was running singles events for Real Live People Party, which I founded in 2004, and had the opportunity to buy NYCBNG in 2007. When the economy tanked at the end of 2009 I decided to stop running singles events and concentrate on running business events. Businesses use down economies to grow, so 18 months ago we launched in Sydney and we are currently under expansion in the United States, hosting events in seven cities.

NYCBNG members are business professionals, executives, small and medium business owners, entrepreneurs and established startups. We provide B2B services through monthly events including networking functions, seminars and expos to help members grow and their businesses.

Eberson: The basis of any good relationship is a win/win for both parties. Allow other people to speak first; the important factor is not who talks?it?s who listens. You have two ears and only one mouth for a reason.

MM: Many young women have a problem deciding what to do after networking events. What would you suggest for follow-up?

Eberson: As you collect business cards, take a minute to jot some notes on the back so it jolts your memory when you follow up after the event. Allow two hours the next day for contacting people.

Social media is a great platform for accelerating the relationship building process. It doesn?t replace face-to-face meetings, it enhances the process. Accused AT&T-iPad hacker Andrew Auernheimer in plea talks (Reuters)

NEW YORK (Reuters) ? Andrew Auernheimer, accused of hacking into AT&T Inc servers and stealing the personal data of 120,000 Apple Inc iPad users, is in talks to plead guilty after his co-defendant did the same last month.

Auernheimer was indicted on July 6 by a Newark, New Jersey federal grand jury on one count of conspiracy to gain unauthorized access to computers and one count of identity theft. His co-defendant Daniel Spitler pleaded guilty on June 23 to the same charges.

In an order on Wednesday, U.S. District Judge Susan Wigenton put Auernheimer's case on hold, saying "plea negotiations are currently in progress and both the United States and the defendant desire additional time to finalize a plea agreement, which would render trial of this matter unnecessary."

Auernheimer is a resident of Fayetteville, Arkansas, and has been free on bail.

Candace Hom, a federal public defender who represents him, did not immediately return requests for comment. A spokeswoman for U.S. Attorney Paul Fishman declined to comment.

Prosecutors said Auernheimer and Spitler were both affiliated with Goatse Security, a group of "Internet trolls" that tries to disrupt online content and services.

The government said both men last June used an "account slurper" designed to match email addresses with so-called "integrated circuit card identifiers" for iPad users.

Once deployed, the slurper conducted a "brute force" attack to extract data about those users, who accessed the Internet through AT&T's network, the government said.

AT&T partners with Apple in the United States to provide wireless service on the iPad. After the hacking, it shut off the feature that allowed email addresses to be obtained.

Spitler could face a 12- to 18-month prison term at his sentencing, scheduled for September 28.

The case is U.S. v. Auernheimer, U.S. District Court, District of New Jersey, No. 11-00470.

US slaps travel restrictions on Russian officials (AP)

WASHINGTON ? The Obama administration says that it has imposed travel restrictions on 60 Russian officials suspected of involvement in the imprisonment and killing of a lawyer.

Sergey Magnitsky died in late 2009 after spending almost a year in jail. He was awaiting trial on tax-evasion charges linked to his work with a U.S.-born investor barred from Russia because of allegations he was a security risk.

Some U.S. lawmakers have been pushing for legislation that would impose a travel ban and asset freeze on the 60 Russians who they believe to be involved in Magnitsky's persecution.

A couple of days ago, I had written about some issues related to the use of?standardised testing?for accountability. ?In an update to the same post, I had mentioned?a statement issued by a gathering of members of Pakistan?s academic, civil, and political society. ?This statement also mentioned the idea of minimum standards that was not the focus of my discussion in the last post. This post is about minimum standards.

The statement?called for formulating the ?minimum standards? for education at all levels and ?measure? them for all children through standardised tests.?In an article contributed to Pakistan Today today, Dr. Faisal Bari elaborated this need for minimum standards with reference to the above-mentioned statement as under:

The provinces need to stipulate the minimum standards of reading, writing, arithmetic, and other areas that are considered to be important. These stipulations will be useless if there is no way of checking if every child, across each province, is achieving these minimums. And since we have both private and public providers of education, it really means measuring the performance of each child.?via?The other side of the coin | pakistanpolicyideas.

Dr. Bari also noted the issues/difficulties with the practice of standardised testing:

There are many issues with standardised testing, and even in places like the US, where standardised testing has been there, in one form or another, for quite some time, there are plenty of critics of the approach: testing tends to be narrow, there can be issues of teaching to tests, it encourages competition and can encourage corruption, it distorts teacher rewards/incentives to a disproportional weight for student performance in tests, it lowers the importance of nontangibles in educational experiences and many other concerns.?via?The other side of the coin | pakistanpolicyideas.

This blog resonates with the concerns expressed by Dr. Bari. They are also noted in earlier posts on the subject of teacher accountability and standardised testing. The issue is not just measuring through the tests though, but doing this in a way that allows for inter-groups comparisons. That is where it is likely to become a bit, in fact extremely, difficult for a country riddled with sharp inequalities.

The other issue is that of minimum standards. ?This perhaps calls for some more deliberation. I have also spoken with friends and colleagues about the idea of minimum standards with some favour in the past. ?Some people also call them Fundamental Quality Levels or (FQLs). ?But it seems to me that there is a danger worth considering when thinking about a minimum standards-based education policy alternatives. ?Or perhaps, it is not as much a danger as a check on the possibility of crafting minimum standards in the presence of another set of standards that are ?not minimum.? If the latter do not exist, then the former will be the only standards and, therefore, not minimum standards. ?It is also possible that prioritising the ??minimum standards? may render them indistinguishable [in practice] from ?the standards.? Requiring uniformly high standards, as is well understood through experience, does not seem to work either in the absence of robust mechanisms to support the individuals and organisation to meet them. ? But it is also difficult to see what kind of incentives for schools and individuals will need to be put in place to make them move up in the empty space [or perhaps one filled with incremental standards-ladder]?between the ?minimum standards,? and ?the standards?? Are we looking at a dilemma re the standards here?

It is possible that the purpose of relying on the minimum standards by the members of the group that met at Harvard was to provide a floor beneath which the quality of services should not be allowed to fall. Yet, if we give priority to minimum standards, then it is also likely that minimum standards become the sticking glue that would not [easily] let most [if not all]?of the?individuals and schools using them to rise above the floor. ?If this line of reasoning makes sense then the minimum standards are less likely to lead to ?educational justice for all,? or they would require its redefinition in a way that is consistent with the existing and uneven nature of an unjust society.

Below, I will give two examples of the use of the term minimum standards in ways that makes them indistinguishable from the standards.

1. The Inter-agency Network for Education in Emergencies (INEE) has a set of minimum standards for INEE. But quite interestingly, those seem to be the only standards. Which effectively means that the minimum standards are ?the standards,? in a situation characterised by emergencies. ?I am not sure how such a situation may be distinguished from the one that prevails in many parts of Pakistan. ?After all, some, and not too far ago, have also talked about an ?education emergency.?

2. ?The second case of the use of the term ?minimum standards,? I pick from my favourite country, the United States. ?This case for ?minimum standards,? was advanced in response to persisting inequalities in California?s education system in the wake of the famous?Serrano v. Priest?case in California?s Supreme Court. ?It requires some more description:

Initiated in 1968 in the?Superior Court of Los Angeles County,?Serrano v. Priest?(John Serrano was a parent of one of several Los Angeles public school students;?Ivy Baker Priest?was theCalifornia State Treasurer?at the time) set forth three causes of action (quotes from the decision).

1. California?s method of funding public education, because of district-to-district disparities, ?fails to meet the requirements of the equal protection clause of the?Fourteenth Amendment?of the United States Constitution and the California Constitution.?

2. ?[As] a direct result of the financing scheme they are required to pay a higher tax rate than [taxpayers] in many other school districts in order to obtain for their children the same or lesser educational opportunities afforded children in those other districts.?

3. ?[That] an actual controversy has arisen and now exists between the parties as to the validity and constitutionality of the financing scheme under the Fourteenth Amendment of the United States Constitution and under the California Constitution.?

The Court agreed with the plaintiffs, largely on equal-protection grounds, and returned the case to the trial court for further proceedings. via?http://en.wikipedia.org/wiki/Serrano_v._Priest

The Serrano cases are a series of cases decided in 1971, 1976, 1977, and 1978, and some of their details can be accessed in the documents above. ?In 1999, nearly 21 years after the case,?Hanif?S. P.?Hirji?reflected on these cases in the?Loyola?Law Review [32 (2)]. This paper spoke of the?minimum standards?as a compromise, since the equality based on an equal distribution of funding to the public schools did not seem to be achievable in a capitalistic society. The case for the minimum standards is made as follows:

California?s current public school system is in desperate need of restructuring to ensure equality in education. The following is a proposal to reduce the impact of the current crisis revolving around educational inequalities.

The first step in achieving this solution is to accept the fact that complete equality can never be achieved in any aspect of our lives. Once we concede that we live in a democratic country that promotes capitalism, we have no choice but to acknowledge that the wealthier constituents will always be capable of spending more on their?children?s educations by placing them in private schools or by?supplementing?their public education through independent programs. This Comment rejects the solutions proposed by Serrano?I?and Serrano II, which sought to equalize funding amongst the various school districts, and instead focuses on establishing minimum standards of education. (p.606)? ?As such, our goal should be to provide each and every student with an education that satisfies the minimum standards promulgated by the people of the state.

The paper further proposed the establishment of a committee in addition to the existing State Education Board:

In conjunction with the State Education Board, the Committee shall organize standardized testing, which should encompass the minimum curriculum adopted by the Committee. These standardized tests shall be administered by independent agencies to all students of?the public schools. No student may proceed to the next grade level without passing the test administered for their present level and each teacher will be paid a bonus based on the number of students in their class that graduate to the next level. The standardized tests will ensure that students are not being allowed to move on to the next grade without having achieved an appropriate degree of proficiency for their current grade level. (p. 607-608)

Notice that though termed?minimum standards?these are effectively proposed as the only, and therefore,?the standards.?

Somali rebels say U.N. food agency still banned, despite pledge (Reuters)

MOGADISHU (Reuters) ? Islamist rebels in Somalia -- who control the parts of the country where famine was declared this week -- have said aid agencies they expelled from those areas last year cannot return, reversing a previous pledge.

The al Qaeda-linked al Shabaab militants imposed a ban on food aid in 2010, which the U.N. and Washington say has worsened the crisis, before appearing to reverse it last week.

The U.N. World Food Program (WFP) was among several groups ordered out of rebel areas which are now preparing to return, describing the situation in Somalia as "extremely dire."

The United Nations told Reuters it had not heard about any new position from the rebels and planned to take last week's pledge at face value and push ahead with food shipments by air and sea.

Al Shabaab had promised to allow relief agencies with "no hidden agendas" greater access to their territory.

"The so-called aid agencies that were already banned and named are not part of the agencies we free to work in al Shabaab areas," al Shabaab spokesman, Ali Mohamud Raghe, told a news conference late Thursday in a rebel-held part of capital Mogadishu.

"They had problems with people and had a hidden agenda. We shall also expel any agency that causes problems for Muslim society," he added, urging hungry Somalis to stay in their homes and wait for the rain to come rather than going to foreign-run refugee camps.

Some 10 million people are affected by famine and drought in a region, dubbed the "triangle of death" by local media, that straddles Kenya, Somalia and Ethiopia.

"WILL BE RISKY"

The U.N. told Reuters it had a "moral imperative" to get back into the areas from which it had been ordered out.

"The approach is to test the ground, to probe and see how far we get," WFP spokesman, David Orr, told Reuters in Nairobi. "We are going to push the operation out unless we hear any different."

"It's not if we go in but when we go in. We know it will be risky but we're going to go with it," he said.

The WFP delivers its food in Somalia through local aid groups and not directly, Orr said.

Al Shabaab accused the United Nations Thursday of exaggerating the severity of the drought gripping the south of the country and of politicizing the humanitarian crisis.

Aid agencies ramped up efforts across the region Friday and intensified international appeals for funding as the crisis began to make international headlines.

The ICRC, one of the very few aid agencies which has kept up its operations in central and southern Somalia, said it had been able to operate throughout the area.

"... we have come to agreements on conditions under which we operate in these areas and it been has (possible) for us to carry out all the operations that we have planned until now and also during the last few months," ICRC spokeswoman Nicole Engelbrecht told reporters in Geneva.

UNICEF, the U.N.'s children agency, were only able to provide care to 35,000 of 640,000 acutely malnourished children in southern Somalia, spokeswoman Shantha Bloemen said.

Nearly 135,000 Somalis have fled since January, mainly to Kenya and Ethiopia. An average of 1,700 and 1,300 Somalis are arriving in Ethiopia and Kenya respectively each day.

The U.N. defines famine as at least 20 percent of households facing extreme food shortages, a crude mortality rate of more than 2 people per 10,000 per day and malnutrition rates of above 30 percent.

Countries including the United States and Britain have stepped up pledges but charities have criticized the response of many governments to funding an estimated $800 million shortfall.
